ADHD Isn’t Just 1 Condition. A New Study Shows What’s Actually Happening in the Brain

Researchers identified three distinct ADHD brain biotypes, each with different neurological wiring, strengths, and risks. That nuance matters—especially for entrepreneurs, where some ADHD traits help, and others can undermine long‑term success.

Though nearly a quarter of entrepreneurs are estimated to have ADHD, the condition isn’t a one-size-fits-all neurotype. Currently, the DSM-5 classifies ADHD individuals into three categories based on observed behavior: inattentive, hyperactive-impulsive, and combined.

In other words, one person with ADHD might be hyperactive and unable to pay attention, while another might appear perfectly calm, but unfocused. Meanwhile a third person with the condition might appear hyperactive and impulsive in one moment and inattentive in the next.

While these categories describe how symptoms appear in the individual themselves, far less research has focused on why ADHD presents this way, and what happens in the brain to cause the divergence.

This complexity means that ongoing research is key to understanding, treating, and even leveraging, the condition. That’s what makes a recent brain imaging study published in the Journal of the American Medical Association, Psychiatry so valuable.

    How the Study Worked

Researchers from the U.S., Australia, and China analyzed the brain scans of 1154 children. (Currently ADHD is classified as a neurodevelopmental disorder that affects children, and the National Institute of Health notes that ADHD is found in about 4.4 percent of adults.)

Like the DSM-5, the JAMA brain study grouped participants into three distinct groups. But rather than grouping people based on the presentation of symptoms alone, researchers looked at how participants’ brains are physically organized and connected, using “morphometric similarity networks.”

This technique compares the physical features of the brain such as volume, thickness, and surface area across different regions. Regions with similar structural characteristics are considered “connected,” revealing distinct patterns of brain organization connected to ADHD.
